Ischemic cardiomyopathy

THIS is my answer to one of my friend regarding  Ischemic cardiomyopathy

Al salam alikum Hassan . this heart condition is very personnel to me as I lost my father and my brother Samir with it,

they are both smokers, and I also lost my Cat Smokey with it believe it or not it can effect pets too, Smokey was diabetic in his last 3 years

and he passed away after his operation , I did everything I could to save him , he passed away at age 13.

Make sure to ask the specialists if there is any other new methods as you need to talk to a good cardiologist and cardiac surgeon specialist.

Ischemic cardiomyopathy (IC) is a condition when your heart muscle is weakened as a result of

a heart attack or coronary artery disease. In coronary artery disease, the arteries that supply

blood to your heart muscle become narrowed.

The left ventricle is enlarged, dilated and weak. This is caused by ischemia –

a lack of blood supply to the heart muscle caused by coronary artery disease and heart attacks.

You will feel

  • Shortness of breath
  • Swelling of the legs and feet =edema
  • Fatigue (feeling overly tired), inability to exercise, or carry out activities as usual
  • Angina / chest pain or pressure that occurs with exercise or physical activity and can also
  • occur with rest or after meals) is a less common symptom
  • Weight gain, cough and congestion related to fluid retention
  • Palpation or fluttering in the chest due to abnormal heart rhythms arrhythmi
  •  Lightheadedness
  • Fainting (caused by irregular heart rhythms, abnormal responses of the blood vessels during exercise, without apparent cause)

The common medications are; Medications to treat myocardial ischemia include:

  • Aspirin. …
  • Nitrates. …
  • Beta blockers. …
  • Calcium channel blockers. …
  • Cholesterol-lowering medications. …
  • Angiotensin-converting enzyme (ACE) inhibitors. …
  • Ranolazine (Ranexa)

As you stopped your Aspirin make sure that your doctor knows that. Remember that the anti cholesterol medications can also cause you to have muscular and joint pain.

Major risk factors of heart disease are a family history, high blood pressure, smoking, diabetes,

high cholesterol, and obesity. can also place one at increased risk for cardiovascular disease

and ischemic cardiomyopathy.

You need chest X-Ray, physical exam by specials, ECG, Echo cardio sonography, blood tests,

CT/ MRI scan of the heart and catheterization and radionuclide study.

The idea is to determine the cause of the cardiomyopathy.

Treatment of ischemic CM is aimed at treating coronary artery disease, improving cardiac function,

and reducing heart failure symptoms. Patients usually take several medications to treat CM.

Medications are used for two reasons:

  1. To improve cardiac function
  2. To treat symptoms and prevent complications

To manage heart failure, most people improve by taking a beta-blocker and ACE inhibitor

even when not having symptoms.

If symptoms occur and/or worsen, digoxin, diuretics, and aldosterone inhibitors may be added.

Other medications will be added as needed. For example, if you have an arrhythmia, your doctor

may give you a medication to control your heart rate or lessen the occurrence of arrhythmia.

Or, blood thinners may be prescribed to prevent complications when a blood clot develops in

the lower heart chamber after a heart attack. Your doctor will discuss what medications are best for you.

You should restrict your intake of salt (sodium) to 2,000 to 3,000 mg per day.

Follow this low-sodium diet even when your symptoms seem to have subsided.

Most salt ingested comes from processed food. In addition to removing the salt shaker

from the table and when cooking, read all food labels for sodium content and serving size

so you can keep track of your sodium intake.

In some patients with advanced heart failure, biventricular pacing

(a pacemaker that senses and initiates heartbeats in the right and left ventricle)

improves survival, reduces symptoms and increases exercise capacity or tolerance.

For people with heart block or some bradycardias (slow heart rates), this pacemaker

will also serve to maintain an adequate heart rate

The ICD constantly monitors the heart rhythm. When it detects a very fast, abnormal heart rhythm,

it delivers energy (shock) to the heart muscle to cause the heart to beat in a normal rhythm again.

After a heart attack, life-threatening ventricular arrhythmias are more common.

This is due to damaged heart muscle. Your doctor will tell you if you meet criteria to have an ICD inserted.

Angioplasty, stents or bypass may be advised to treat coronary artery disease.

In addition, some patients may be candidates for surgical remodeling or reconstruction

of the left ventricle or left ventricular assist

Regarding your Anemia and that is a condition where there are a reduced number of red

blood cells or hemoglobin in the blood or a dysfunctional RBC. There are several types of anemia,

but the most common one is caused due to deficiency of iron in the body. Taken diabetes medication

can lower your iron, if you have bowel problems or liver problems can do that too.

Make sure you take the Iron only when your doctor advice you to take it as it can damage the liver in the long run.

Too much tea and caffeine, or smoking can also reduce your iron.

Dandelions contain beta carotene, which is an antioxidant that helps protect cells from damage.

Carotenoids such as beta-carotene play a vital role in reducing cell damage.

The flower of the dandelion is also full of polyphenols, which are another type of antioxidant.

Dandelions contain bioactive compounds that may help lower a person’s cholesterol.

It reduced total cholesterol and levels of fat in the liver. The researchers concluded

that dandelion might one day help treat obesity-related nonalcoholic fatty liver disease.

There is some evidence to suggest that dandelions contain compounds

that may help with regulating blood sugar.

Some studies indicate that dandelion extracts and compounds may

help reduce inflammation in the body.

However, dandelions are a good source of potassium, there is clinical evidence that shows

that potassium can help reduce blood pressure.

Some researchers have proposed that dandelion could help people achieve their weight loss goals.

This is based on the plant’s ability to improve carbohydrate metabolism and reduce fat absorption.

There is growing evidence that suggests that dandelions can help boost the immune system.

Some research indicates that dandelion may help protect the skin from sun damage, and aid the digestive system.

It is not advisable to consume dandelions from the backyard.

Dandelion leaves are sometimes present in salads, but they are not widely available in all areas.

Picking dandelions in a backyard is unsafe due to the potential presence of pesticides or animal

Dandelion supplements can cause allergic reactions in some people.

People should not use dandelion supplements if they are sensitive to dandelions or certain other plants,

such as ragweed, daisies, or chrysanthemums, or marigolds.

People trying supplements should follow instructions on the bottle for recommended doses

and always speak to a doctor before taking them.

Giddiness

Dear Dr Saad,

This Hassan again follow up from WA brief communication earlier.

Due to time difference it’s wise we communicate via email to enable each of us attend at its time of convenience.

Ok I’m being diagnosed Ischemic Cardiomyopathy, (got my 2nd heart attack on 9th May 2021) then on 12th Jul came back to hospital due to fluid overload. Warded and treated over a week then discharged.  Early Jan 2022 came back with Edema but Dr just provide medication which doesn’t go away and I came back to A&E 1 mth later and again just increae the dosage (now @ max per day consuming) over and above during this period via results of blood test I also got Anemia.

I believe that those medicines prescribed are subject to side effects (this warning clearly stipulated at the packaging) consequence my kidney also begin to deteriorate (already schedule for renal review mid Mar). They’ve change some of the medicines and stop taking Aspirin. But I’m still concerned on slow recover on Edema and is now supplement with Dandelion base capsule which shows better urination discharge in volume and timing. Meanwhile need to address the Anemia situation with probably herb base supplement or alternative medicine .
